This Excel tracker is set up for a fictional youth arts nonprofit that runs programs at twenty school sites. Session form responses roll up by site, month, and facilitator, sites that have not reported are flagged, and a funder report fills itself for the quarter chosen.

Open larger imageFacilitators submit a short form after each session, and the responses land on a session log laid out like a form response sheet. The workbook rolls those responses up by school site, by month, and by facilitator, and tracks attendance rate and average student reflection score across the school year with native line charts. A site rollup compares sessions logged in the reporting month with sessions planned and marks each site as up to date, behind, or missing reports.
The session log flags duplicate submissions, attendance typed higher than enrollment, and reflection scores outside 1 to 5, and keeps duplicates and out-of-range scores out of the totals. The funder report sheet produces a one-page quarterly summary, including a paragraph that writes itself from the numbers, and the checks sheet confirms every rollup agrees with the log.

This Excel workbook consolidates five hospitals' community benefit activity logs into one dataset and rolls it up to the lines of IRS Form 990, Schedule H. Every total on the summary ties back through formulas to the row a hospital reported.
